13 weeks | Days | 8-hour shift | 40 hours weekly

Incumbent serves as an Outpatient/Inpatient Social Worker/Registered Nurse (RN) Case Manager and is responsible for direct supervision and supportive contact for a specific group of identified high-risk patients. The Social Worker/RN Case Manager is responsible for designing and managing a continuum of care focusing on empowering clients to achieve demonstrable outcomes and self-sufficiency. The Social Worker/RN Case Manager is responsible for assessment, service planning, resource acquisition, monitoring progress, and initiating and responding to emerging client needs. The Social Worker/RN Case Manager will link the client with Community Social Service providers, health care providers, substance abuse, and mental health providers to achieve specific goals.

The Social Worker/RN Case Manager is responsible for utilization review of patient cases in the inpatient and outpatient services departments, and review of medical necessity of referrals to specialty providers for services not normally provided within the facility. The incumbent will work with a multidisciplinary team to promote the efficient and effective use of a variety of health care resources by focusing on the whole patient to provide a seamless continuum of care. The goals are the provision of quality care, decreasing the fragmentation of care, enhancement of patient self-care and quality of life, and cost containment. The incumbent will work closely with Providers, Nurses, Social Workers, Physicians, the PRC department, and all healthcare providers' staff to ensure implementation of appropriate resources in the fulfillment of utilization goals for the hospital and third-party payors. The Social Worker/RN Case Manager will report to the Director of Case Management/Care Coordination.

EXPERIENCE/EDUCATION/QUALIFICATION

Experience:

• Must have a minimum of three (3) years of experience in the clinical care setting

Education:

• MSW Social Worker:
• Master's Degree in Social Work (MSW)

Licenses, Certifications, and/or Registration:

• Must have and maintain current Basic Life Support (BLS) certification by the American Heart Association throughout employment
• Licensed Master Social Worker (LMSW).
• A valid, current, full, and unrestricted Professional License to practice LMSW or LCSW in any state of the United States of America, the Commonwealth of Puerto Rico, or a territory of the United States

EMR System: Allscripts experience preferred

Shift: Monday to Friday, 8 am-5 p.m. Weekend on call rotation, would be 1 weekend a month

Float Requirement: Travelers are occasionally expected to float to other departments in which they are qualified to work. This could include taking a patient load if experienced/competent in that setting, or could be task-oriented, such as answering call lights, phones, etc

Orientation: on Mondays only - If your traveler misses the compliance deadline, they will push to the following Monday

COVID & FLU EXEMPTIONS: May accept after declination form is submitted

HOUSING APP: Must be submitted 2 weeks prior to start

Knowledge/Skills/Abilities:

• Experience and knowledge of discharge planning, case management, or utilization review processes.
• Demonstrate knowledge of electronic health record systems.
