Title: Per Diem CORE Clinician
Reports to: Assistant Director of Social Programs
Job Objectives:
· Carry a caseload of CORE CPST clients, providing comprehensive support and guidance to individuals facing behavioral, emotional, and/or educational challenges.
· Conduct intakes and clinical assessments for MHA programs, ensuring that individuals receive appropriate and personalized care.
Responsibilities:
CORE CPST Caseload Management:
· Provide individualized support and intervention services to clients enrolled in the CORE CPST program.
· Conduct comprehensive assessments to identify clients' strengths, needs, and goals.
· Develop and implement treatment plans in collaboration with clients, their families, and other involved professionals.
· Monitor progress and adjust treatment plans as necessary to ensure clients' ongoing well-being and success.
Intakes and Clinical Assessments:
· Conduct intake interviews and clinical assessments for individuals seeking services.
· Evaluate clients' mental health, social, and emotional needs to determine appropriate levels of care and intervention.
· Collaborate with interdisciplinary teams to develop comprehensive treatment plans that address clients' holistic needs.
· Complete assessments and evaluations for Supported Housing in accordance with Local Professional Health Authority (LPHA) recommendations.
Other Duties:
· Answer MHA's main office phone as needed.
· Participate in agency committees as needed.

Requirements:
Qualifications:
· Master's degree in social work from an accredited institution.
· Current LMSW licensure in NY.
· Minimum of 2 years of experience providing direct clinical services to individuals with behavioral, emotional, or educational challenges.
· Experience working with children, adults, and families in various settings, including community mental health, residential care, or similar environments.
· Strong assessment, diagnostic, and treatment planning skills.
· Excellent communication, collaboration, and interpersonal skills.
· Knowledge of evidence-based practices in mental health treatment.
· Familiarity with relevant laws, regulations, and ethical standards governing social work practice.
· Training or certification in specialized interventions for specific populations strongly preferred (e.g., trauma-informed care, dialectical behavior therapy, etc.).
· Ability to work effectively within a multidisciplinary team and independently.
· Commitment to cultural humility, diversity, and inclusion in service delivery.
· Spanish-speaking strongly preferred.
