Job Description

About the Company:

We are seeking a dedicated Pharmacist with knowledge and proficiency in pharmacy practice including pharmaceutics, pharmacokinetics, pharmacodynamics and pharmacotherapeutics for the C. W. Williams Community Health Center, Inc. The pharmacist is responsible for interpreting and evaluating appropriateness of pharmacological therapy to ensure that patients receive optimal drug therapy.

About the Role:

The pharmacist is responsible for providing a full range of pharmaceutical service functions including, but not limited to:

Clinical:

  • Select appropriate drug therapy based upon the pharmaceutical principles of pharmacokinetics and pharmacodynamics
  • Complete patient-specific monitoring for efficacy, adverse effects, and clinical outcomes
  • Communicate relevant findings and/or recommendations to other health care providers verbally and in writing
  • Provide medication and device counseling to patients and caregivers
  • Complete medical history and medication reconciliations
  • Establish a patient panel for continual follow up under a CPA

Education:

  • Provide concise, applicable, comprehensive, and timely responses to requests for drug information
  • Educate health care professionals, students, patients, and the public regarding rational drug therapy
  • Maintain basic knowledge of pharmaceutics, pharmacokinetics, pharmacodynamics, and pharmacotherapeutics in the area of practice
  • Establish an understanding of the 340B program and help to grow pharmacy utilization
  • Contribute to quality improvement efforts and develop new initiatives, policies, and procedures
  • Complete Chronic Care Management consults

Administrative:

  • Establish and implement programs that help ensure cost-effective drug therapy and successful patient outcomes
  • Manage continuous glucose monitor initiative
  • Implement policies, practices, and procedures for clinical pharmacy services, ensuring compliance with regulatory standards
  • Collaborate with Director of Pharmacy to ensure optimal allocation of 340b resources

Qualifications:

  • Education: Doctor of Pharmacy degree from an accredited School/College of Pharmacy
  • Experience: post-graduate year one residency or equivalent experience in ambulatory care or rural health
  • Licensure: current in-good-standing or eligibility for NC pharmacist license and clinical pharmacist practitioner license
  • Other: NC driver’s license for between clinic driving as necessary skills
  • Prior knowledge of local, state, and federal regulations
  • Free of chemical use problems for two years immediately preceding hire
